1. Birth certificate

2. Immunization records

3. Proof of residence

   • current Conectiv bill (no greater than 60 days old) - or -

   • lease agreement - or -

   • settlement papers

4. Social Security number of student

5. Parent(s) photo identification

6. Most recent report card or progress report if possible

7. If there is a custody issue involved, court papers and/or guardianship papers are needed.
