A doctor’s action plan of care for your child listing:

Þ Current status of health condition

Þ Medications student takes

Þ Treatments needed

Þ Emergency Intervention Plan

Routine and Emergency Medication

Þ Must have prescription label

Þ Permission slip from parent, signed and dated

Þ All Asthmatics must have emergency inhalers in school

Equipment and supplies

Þ  Asthma—nebulizers, tubing, spacers              

Þ Diabetes—food, one-touch machine, test strips, lancets, pump supplies

                   **Medications and Supplies should be replaced as needed**
