What do I need to do if my child
needs medication at school?
I will need to receive a signed and dated
note from a parent/guardian requesting that
medication be administered at school. The
name of the medication, dosage and time to
be administered should also be indicated on
the note. The medication will need to be in
the original labeled container with the
student’s name on the container.

What should I do if my child has
asthma and needs his inhaler at school?
Please send a note to me indicating that
your child has asthma and needs to take his
inhaler at school. Also indicate the dosage
and times that he/she will need to take it.
The inhaler must be in the original labeled
box with your child’s name on the label.
If the doctor has given you a sample
inhaler there will be no label on the box.
Please ask the doctor to write a
prescription note including the child’s
name, the name of the inhaler, the dosage,
and the times that the inhaler will need to
be given.

When am I required to keep my child
home from school if they are not
feeling
well?
If your child has a temperature of 99.5
degrees Fahrenheit when he/she wakes up in
the morning, he/she should not come to
school. If your child has been home sick,
he/she should not return until they have
been fever free for 24 hours.
- What
information would you need if my child
gets an injury requiring medical attention
outside of school (especially broken bones
and stitches)?
If your child becomes injured outside
of school and the injury requires medical
attention, I would need a note from the
doctor specifying the type of injury and
any restrictions that your child would
have while in school.

If my child has been sick for several days,
does he/she need anything when returning to
school?
If you child has been absent 3 or more
consecutive days, a doctor's note indicating
the reason for the absence is required.
For shorter absences, a parent note will be
fine.

If my child receives medication during the
school day, what happens when he/she goes on
a field trip?
You should contact your child's teacher to
make arrangements for him/her to receive
their medication on the field trip. If
you wish for your child to receive his/her
medication upon returning from the field
trip, a note indicating this should be sent
in to the school nurse.
