Prior to any Assistive Technology Referral, ask these questions:

 

What task does the student need to do that is currently difficult or impossible? Why?  What strategies need to be changed?  What AT may be helpful?

New Procedures as of January 2009

Prior to any Assistive Technology Referral, consultation with an Inclusion Coach is necessary.

bullet

All AT Referrals require pre- and post-Implementation Plans, including supporting and on-going data collection.

bullet

Refer to AT Referral FLOW CHART or Page 1 of the Referral for further instructions.

Use the forms and charts below to help guide the IEP or 504 team in determining whether a referral to an Inclusion Coach for these purposes would be helpful.  These will also help in making an AT Referral.

 

New Procedures as of January 2009

All REACH requests regarding positioning equipment will be directed to the REACH office.

bullet

All non-REACH requests are to be directed to the AT department.

bullet

All Permissions to view Stubbs and Requests to transfer equipment are to be directed to the REACH office.

All requests require consultation with Occupational, Physical and Speech Therapists.
